Department of Neurology, Chiba Emergency Medical Center. Epstein-Barr virus (EBV) infection is occasionally accompanied by acute neurological impairment. The pathogenesis of neurological manifestations with EBV infection consists of primary inflammations of EBV infection, and secondary immunologic reactions. However, their clinical course and prognosis are usually favorable. Here we report a patient with fulminant neurological involvement in association with EBV infection. The patient was a 44-year-old man. One morning he developed ataxic gait and speech following flu-like symptoms. He noticed double vision in the afternoon. He had disturbance of consciousness, bilateral ptosis with mydriasis, opthalmoplegia, facial diplegia, bulbar palsy, and weakness of muscles in extremities and respiratory system on the next day. He required mechanical ventilatory support for a month. His symptoms began to improve gradually two weeks after the onset. Two month later, neurological examinations disclosed severe cerebellar ataxia of the four extremities and ocular movement, cerebellar speech, and moderate weakness in his limbs. Moderate cerebellar ataxia and diminished deep tendon reflexes remained for 8-months. Although he had no physical manifestations of infectious mononucleosis, DNA of EBV was identified in the cerebrospinal fluid (CSF) by the polymerase chain reaction method. From these results, we diagnosed his condition as a cerebello brainstem encephalitis with polyradiculitis associated with EBV infection. The cell counts and protein content of CSF gradually normalized in the early stage of his illness, but CSF protein increased again, and had the peak of 275 mg/dl in about one month. In spite of normalized CSF cell counts, his neurological symptoms persisted. CT scan and MRI studies of the brain and the spinal cord were repeated, but demonstrated no significant abnormalities. Clinical course and CSF findings revealed that his fulminant neurological symptoms were most likely produced by the secondary immunologic reactions following the primary inflammations by EBV infection. Acute cerebellitis is a rare complication of Epstein-Barr virus (EBV) infection. We report the case of a 21-year-old woman with a 12-day history of nausea and vomiting, gait and limbs ataxia, myoclonus, tremor of head and all four limbs, opsoclonus and cutaneous rash. Anti-EBV IgG and IgM antibodies against antiviral capsid were positive and anti-EBV against virus-associated nuclear antigen was also positive. wow, I had mono when I was about 26, 3 years after
a severe bout with Toxoplasmosis... I remember that not
long after the mono incident, during which I spent many
hours lying on the bed breathing with difficulty, I
started having lots of pain in my left shoulder/neck
area. I still have this pain sometimes really really
horribly bad, other times it just remindes me it's still
there. Things really just got worse for me after that
mono. That was about 20 years ago. Does that seem right
or am I imagining all these correlations because of what
I have read here?

Peggy I missed this earlier. Hah this one is a
whopper isn't it with the science words? I guess only a
doctor could explain it, but what I was looking for was
evidence that yes, indeed chronic mono [which in me are
FMS symptoms], might be a result of this. When I was in
my worst flare I had all the symptoms of a neuro/spinal
encephalitis going on. That can come from many things, or
many bacterias or viruses. In my wandering thru abstracts
I do see some inferrence of EBV mono inflamation and many
other herpes viruses causing acute muscle pain and
weakness, etc. This is probably rare, but how many never
go to a doctor and think they just have a horrible flu? I
was so ill for months. And my neck and spine pain was on
fire. I thought that was a separate issue, as most
doctors do. But it's possible that it could have been the
reactivation of EBV virus that caused all the pain and
neuro problems. [similar to this guy above.]

Hi Herne, I think what you are saying is very
typical of active EBV.

FMS - Infectous Mono | #9 | ||
|
I am 48 year old female with FMS and I just came
down with a case of Infectous Mono. I know some of the
symtoms, but my question is has anyone had Mono and
started to feel better after a week and a half or two,
and then started to feel ill again? I am not feeling
quite as ill as the first week but it is a set back. Any
feedback would be a big help. Thanks

Hi Ncrocker, yes I have had it return. In fact it
continues to return unless I stay in bed for quite a few
days. Rest makes me feel better and as soon as I have a
long day, it returns. I believe that is why they call it
Chronic EBV. Why it happens I have no idea, but it
coincides with my FMS symptoms. Actually I think they are
all related. You will read about it more in the CFS
forums, but not so much in the FMS world, although I do
see the same symptoms in both catagories.

Thanks, Linda - wow - all that from EBV!!
Now that I know I have Lyme, it doesn't change my opinion that my chronic EBV is probably still a big culprit in my health, especially my spinal issues. My poor immune system can't cope with both organisms running amuck. Each organism probably helped the other one proliferate, I suspect. Well, that is theory #265. Thanks - Tracy

![]() I was at U of Georgia and one day I just stopped going to classes and slept,thats all I remember I didnt eat,just stumbled down the hall to the dormitory bathroom(at least I guess I did),I slept and slept the ironic thing was that my roommate had moved out to stay w someone in her yr and I was alone after 2wk my psychology prof,w whom I had a good relationship,wondered where I was and asked a couple of my classmates to go by my room its all very vague,but I remember having blood tests,with"bizarre"results the dr said(Oh god I have leukemia Im dying) after about a month back home I was much better--no treatment,just rest--and told that mono was a 1-time thing but my Oh Im dying periods have come back periodically,often around March,and Ill have blood work done--but I havent tested for ebv another interesting thing--those neuro symptoms--I had them in 1974 and 1998--the first time diagnosed as labyrinthitis,the second as leukoencepatopathy(vascular),or swiss-chees brain(maybe to go w my swiss-cheese tendons?) very interesting thread! |
